Chronic myelomonocytic leukaemia, type 1 (CMML-1)

PB MGG (1000×)

Left to right: two moderately mature neutrophilic granulocytes with decreased granulation (a metamyelocyte and most probably a myelocyte with fine cytoplasmic vacuolation and delayed segmentation of nucleus) and one slightly atypical monocyte.

PB MGG (1000×)

One less mature monocyte – a promonocyte.

BM MGG (200×)

Marrow aspirate smear with an adequate cellularity and an almost proportional representation of haematopoietic cells. A megakaryocyte of a normal size and with normal lobulated nuclei is seen on the left (yellow arrow).

BM MGG (1000×)

Atypical monocytes (i.e. those with decreased lobularity of the nucleus) are marked by red arrows. Other cells seen in the image are neutrophils in various stages of maturation, all of them showing decreased granulation.

BM MGG (1000×)

Atypical monocytes are marked by red arrows, a cluster of four borderline polychromatic erythroblasts is marked by a green arrow on the right, and one standalone erythroblast is marked by another green arrow on the left bottom. The black arrow shows a granular blast.

BM ANBE (1000×)

Alpha-naphthyl butyrate esterase (ANBE) staining: monocytes (red arrows) stain with various intensity, the reaction product is brown and diffuse. Other cells shown in the image (an erythroblast and neutrophils) do not stain in this method.
