Refractory cytopenia of childhood

BM MGG (200×)

The bone marrow aspirate smear is markedly hypocellular, with sporadic haematopoietic cells. This is a possible finding in RCC; however, the evaluation of an adequate bone marrow trephine biopsy specimen is essential for diagnosis.

BM MGG (1000×)

One dysplastic macroerythroblast with a pyknotic nucleus (yellow arrow), two normal lymphocytes (blue arrow) and one unmarked promyelocyte with an atypical kidney-shaped nucleus.

BM MGG (1000×)

Four dysplastic erythroblasts are shown in this image. One erythroblast is moderately macrocytic and its nucleus is slightly constricted (yellow arrow), one erythroblast is macrocytic with an indication of karyorrhexis (black arrow) and internuclear bridging is seen between the two remaining erythroblasts (green arrow).

BM MGG (1000×)

One dysplastic macroerythroblast with a pyknotic nucleus (yellow arrow), basophilic stippling and dehaemoglobinised cytoplasm. Two neutrophils with coarser granules are seen on the left side of this image.

BM iron stain (1000×)

One sideroblast (red arrow) with pathological clumping of a larger amount of siderophilous granules.
