Chronic myelomonocytic leukaemia, type 0 (CMML-0)

PB MGG (1000×)

Three mature monocytes, one segmented neutrophil (white arrow) and numerous platelets showing anisocytosis.

BM MGG (1000×)

Slightly atypical monocytes (i.e. those with decreased lobularity of the nucleus) are marked by red arrows. Furthermore, the image shows a large mature megakaryocyte with a non-lobulated nucleus (yellow arrow), slightly megaloblastoid metamyelocytes and band cells (green arrows) and an eosinophilic metamyelocyte with basophilic progranules (black arrow). A large bare nucleus of a megakaryocyte with platelets nearby is seen on the right.

BM MGG (1000×)

Atypical monocytes are marked by red arrows. Furthermore, the image shows one macrophage (black arrow), four polychromatophilic microcytic erythroblasts (yellow arrows), one neutrophilic band cell (green solid arrow), multiple borderline hypersegmented mature neutrophils (green dashed arrows) and lymphocytes (unmarked cells).

BM MGG (1000×)

Atypical monocytes are marked by red arrows, two dysplastic megakaryocytes are marked by yellow arrows; the one on the bottom is smaller and binucleate. Other cells shown in the image are neutrophils in different stages of maturation.
